Insurance Information – Medical Conditions

Our practice accepts the following insurance plans for the evaluation and treatment of medical conditions.

Please call (954) 727-5700 for insurance verification and to learn about our Out of Network options for insurance plans that we do not participate with.

 

Payment for any outstanding balance and co-payments is due at the time of service. Health insurance plans do not cover procedures that are not medically necessary, which includes all cosmetic procedures.

 

Securing referrals is the responsibility of the patient. The office must be in receipt of the referral prior to your visit. If a claim is denied due to an invalid, incorrect, or missing referral, the patient is responsible for the cost of the visit.

 

Insurance plans have variable coverage and may provide only partial reimbursement of our fees.

Insurance may not provide reimbursement for any unmet deductibles.

 

The remaining balance is patient responsibility, and a credit card held securely on file is required. It is important to understand what your insurance plan entails.

 

If we do not participate with your insurance plan but your plan has “out of network” benefits, we are happy to provide the necessary documentation for submitting an “out of network” claim.

 

If we do not participate with your plan, you will be responsible for full payment at the time of service.

 

 

What to Bring


For all patients at each visit, please bring your photo ID and insurance card.

 

For New Patients – Cosmetic Consultation

 

If you are interested in receiving treatment the same day as your one-on-one cosmetic consultation, please let us know when scheduling the appointment. We will make every effort to accommodate your schedule.

 

The fee for new patient cosmetic consultation is $150, which can be credited towards your cosmetic treatment performed within 60 days of the consultation.

 

 

Before Your Visit


With any medical grade cosmetic treatment, we want to ensure you have an optimal experience. This is achieved by simple tips to prepare for the procedure.

 

With any light, laser, chemical peel or microneedling treatment, practice strict sun protection (avoiding any suntan or sunburn) prior to the procedure.

 

Eat breakfast and drink adequate amounts of water prior to the appointment.

 

Avoid alcohol for three days before your treatment.

 

If medically approved, avoid Aspirin/Ibuprofen/NSAIDs, vitamin E, omega 3 and fish oil supplements 10 days prior to treatment.

 

Take arnica or bromelain tablets as directed two days before and continue after treatment. You can find these tablets at most health food stores.

 

Don’t plan a work-out class or massage immediately after the treatment.

 

 

Checking In


After booking an appointment with us, patients can login to our secured web portal to input or update your contact information and health history. For new appointments, please arrive 15 minutes prior to your scheduled appointment time to fill out our electronic intake forms. For all our existing patients, we ask that you arrive 5 minutes early to register, update your information, and enjoy our space.

 

 

Cancellation, Missed Appointments, and Late Arrivals Policy

 

Ayana Dermatology is committed to providing all of our patients with the absolute best care. When an appointment is scheduled, that is time set aside by the provider for you, and when it is missed, it cannot be used to treat other patients. No shows, late arrivals, and last minute cancellations prevent another patient from being cared for.

 

Ayana Dermatology reserves the right to reschedule late arrivals of more than 15 minutes.

 

In an attempt to be consistent with this and fair to all of our patients, we have a firm Cancellation Policy.

 

We require that you give our office a 24-hour notice in the event that you need to reschedule or cancel your appointment.

If you miss an appointment without contacting our office at least 24-hours prior to the appointment, you will be subject to a non-refundable cancellation fee.
